Becoming a Life Coast Health Patient

Life Coast Community Health Center cares for patients in Thibodaux and Houma. The steps below walk you through what to do first. Appointment availability and what is required for your visit are best answered by our staff, so please call and ask.

  4. 4. Get ready for your visit

The providers and visit types shown there are the ones currently open for online scheduling, so you may not find every provider or service listed. For any other provider, location or service, call the clinic and our staff will help you schedule.

What to Bring to Your First Visit

  • Driver’s license or proof of identification
  • Proof of income
  • Proof of current insurance
  • Proof of address
  • Co-pay
  • Current medications and immunization records.

Cost of Care

Insurance & Help Paying for Care

We accept most private insurance plans, including Medicare and Medicaid, as well as cash, check and credit cards. If you are uninsured or having difficulty paying a bill, tell a Patient Service Representative. Our staff can help you look at applying for insurance, our sliding fee discount program, or a payment plan.

Sliding fee discounts are based on income and household size, and required documents must be submitted. Call us to ask what applies to your situation.
